The National Provider ID Data Template - Splitting Existing National Providers
This form supports the correction of National Provider information, where a number of local providers have been grouped together as one National Provider, when in fact they represent different provider organisations. I.e. a single National Provider needs to be split into two or more National Providers.The form should be completed and issued to the SP Helpdesk.
