Abstract: This and the accompanying guidance provide lead officers with a route map through the raft of health policies, targets and aims/objectives. It sets out for the key client groups the overlaps and strategic links between health agencies and supporting People services.

The guidance condenses 800 pages of health guidance into bite sized chunks which can provide an easy framework to make links to the appropriate commissioners of services and start to share common aims and objectives which will enable joint packages of health/social care, housing and support.

Many services already exist which deliver against the key targets outlined in the guidance. The local Commissioning body needs to make best use of the services which it will have established contracts for with 1st April 2003. A parallel pack will go out to each health agency (PCT and Strategic Health Authority) to ensure joint understanding of how the links can be made and to which targets future joint work can deliver.
