Department for Health Partnerships for Older People Projects

The attached document sets out information on a series of pilots which the Department of Health intend to develop and roll out around coordinating care for vulnerable older people. Supporting People teams will have an interest in ensuring that the local programme is recognised and plugged into any proposal which their authority may wish to develop. ODPM is represented on the steering group for this work and will ensure that Supporting People and broader housing interests continue to be picked up as the pilots are further developed.
